Comments, miscellaneous information that applies specifically to these types of information, can be entered right along with the specific records themselves. One crucial use for these Comment fields is to verify any unexpected information such as a preference for a nickname or confirmation of alternate spelling of information. Note: Data processors, whenever you modify any name field to reflect the specific request of the entity, note that in the comment. Include the actual requested name format, the word “request” or “prefer” and your name and date. For example, “Entity requests Miss Betsy B. Bain per phone call. C.
