Applicants must provide three (3) types of acceptable identification from the list below to prove their identity when applying for a National Criminal History Record Check. Identification must originals, not be expired and should include at least one type of photographic ID and identification that contains a signature and date of birth. Applicants must record in the space provided in the application form the identification type (eg Medicare card) and number (if applicable).
