What application form do I need to file to pickup and deliver property (including household goods or passengers) for-hire between points in Missouri?
You will need to complete a MO-1 application form, file proof of insurance (Form E) and have systems and procedures in place to comply with the safety regulations. You must enclose $10 for each vehicle you plan to operate within this state unless fees have been paid under other authority.
