What address was the 1098-T form mailed to?
Per federal regulations, this form was mailed to your permanent address on file in the UH student registration system. If there was no permanent address, then the form was mailed to your mailing address or your current residence address. If your permanent address was incorrect, and you did not receive a 1098T form as a result, please take the following steps: • Go to http://www.hawaii.edu/1098t and print your 1098T form. • Download the Student Data Change Form from http://www.hawaii.edu/admrec and mail in or take the form with updated information to your home institutions records office. You cannot update your permanent address on-line, only mailing addresses can be updated on-line. Note: It is very important that you update your permanent address in the student registration system any time there is a change.
