We will be out of town for the summer. Can I have my childs school entry physical exam performed outside of Florida?
• School entry physical exams must be performed by a physician in the United States. A student may have a physical performed in the United States outside of Florida, but must be completed on the yellow DH 3040 State of Florida School Entry Health Exam form obtained from a Florida physician or the Florida Department of Health.
