New vulnerabilities keep cropping up frequently. Also there may be vulnerabilities which may have been missed out in an assessment. A small vulnerability left open may allow an attacker to infect a website. Apart from periodic vulnerability assessment and penetration test, it is better to augment it with a proactive and continuous monitoring of websites for malware.
