There is no specific format for public interest waivers. You should include as much information as you believe is necessary to allow EERE to make the proper determination. For waivers based on unreasonable cost, DOE has published guidance (PDF 34 KB). All waiver requests should be coordinated with the Project Officer and submitted by email to BuyAmerican@ee.doe.gov.
