UPIN and the supervising/attending UPIN?
A5. The referring/ordering UPIN is reported in Item 17a on the HCFA 1500 claim form. This represents the physician or other practitioner who referred the patient to a specialist or ordered tests or other services. The ordering UPIN is required for claims containing the following types of services: Diagnostic Laboratory Diagnostic Radiology Consultations Durable Medical Equipment The supervising/attending UPIN, on the other hand, is reported in Item 19 on the HCFA 1500 claim form. This UPIN represents the physician or other practitioner who is the patient’s primary doctor. The supervising/attending UPIN is required when the patient is receiving physical and occupational therapy and/or routine foot care services. The patient’s primary care doctor must see the patient every 30 days to update their plan of treatment. Electronic claims must also show the two types of UPIN described above in the appropriate electronic fields when needed. Q6.
