Top 13. How Can I Get a Payment or a Voucher for Free Repair Service?
If you fall into one of the two categories described in Question 6, you may fill out and mail in a Claim Form to ask for one or both of the benefits for which you can prove you are eligible. A Claim Form is available on this website. You can get additional Claim Forms by calling 1-800-207-0343. The Claim Form asks you for information about your Subject Oven and for proof of any Qualifying Repairs. Follow the instructions on the Claim Form closely. You may not submit a claim or supporting proof online. You must sign the Claim Form under penalty of perjury and mail it to the Settlement Administrator. Submitting a fraudulent claim is against the law. Your Claim Form must be postmarked or actually received by the Settlement Administrator no later than April 11, 2008. If you miss the deadline, you will lose your right to compensation under the settlement.
