It is up to the Chief Nurse Officer to determine if an error or instance of conduct reaches the level of concern that would suggest an official report to the Office of Professional Discipline or to the Department of Health. There are three events regarding medication errors that require a report to the Department of Health’s patient tracking system (NYPORT system): permanent patient harm, near death as evidenced by cardiac or respiratory arrest, and death.
