You can apply to your Internet Service Provider (ISP) or directly to us. Fill-in and submit the application available on the US Domain Web site. Be sure to read all instructions carefully. Do not alter the application and be sure to fill it in completely and correctly. • Make sure that the name you are requesting follows the naming structure (“www” should not be entered as part of the domain name). • Indicate “New” as the registration type in line one of the application. • We no longer support or use “NIC-handles". • Fill in all other lines and do not abbreviate or use “same.” • Put “Request for new domain: my-domain.city-name.state.us” in the subject field. • Submit the application via email to support.us@neustar.us. • Be sure your name servers are prepared BEFORE you submit the application. Applications are handled automatically and it is important to follow these instructions exactly. Every error delays your application and the applications of others.
