You need to “add” a new T4 slip by clicking the “Add new” button in the navigation tool bar with a “plus” icon. By default, the navigation tool bar is the 2nd tool bar from the top. If you cannot see this tool bar, you can show or hide the tool bar via menu: View > Toolbars > Navigator.
