For the purpose of measuring days between the enquiry and completion of the care plan, the care plan is authorised when who ever has the authority i.e. the budget holder or team leader etc has signed the care plan off. The Sign off stage will vary depending on the care plan but will occur when the care plan is ready to be put into place. Q: How do we define a ‘New Care Plan’? For instance if a client who currently has a care plan, goes into hospital and due to a large change in circumstances, when they come out of hospital they receive a new care plan. Should this be included? A: Care plans arising from reviews should be excluded.
