The Explanation of Benefits (EOB) document contains additional patient information not associated with the administration of the dose of GARDASIL. Is that OK?
The Explanation of Benefits (EOB) that is submitted with the rebate form should contain only the patient’s own health information. It is acceptable to black out any additional patient information on the EOB that is not associated with the cost of the dose of GARDASIL. All patient information contained in the EOB, whether associated with the administration of GARDASIL or not, will be held in strict confidence by Merck and the third-party administrator.
