A20: If the children listed on the Notice are the same children covered by the employee’s insurance, the employer or the plan administrator should complete the insurance information on Part B of the Notice and return it to DOR. If any children listed on the Notice are not covered by the employee’s insurance, the employer must add them to the health insurance plan and then return the Notice to DOR.
