Please try the following steps and see if it works. • Open Outlook • Select the Tools menu • Click on Options. • Click on Other. • Click On Advanced Options. • Click On Addin-Manager. • If Email Questionnaire Outlook Add-in is not checked, check the box and restart Outlook If this does not work, please try the following method again: • On the Outlook menu, Help=>About. • Check if there is a “Disabled Items”. If there is, click Disabled Items. • See if “Email Questionnaire add-in” appears . • Select the add-in and click Enable.
