The care team is an essential component of a medical home. How does the PCMH Survey include patients experiences with the care team?
In order to capture the concept of the care team, some of the items in the field test version of the CAHPS PCMH Survey ask about experiences with this provider or anyone in this providers office. The CAHPS Consortium found in cognitive testing that some patients defined care team to mean both providers in the practice as well as providers in external practices that deliver care to the patient. To minimize the possibility of confusion, we replaced the term care team with anyone in this providers office.
