Temperament Theory Course – I was instructed to fill out a Personal APS survey form, but there was no Scantron Card. What do I do?

Students taking this course as a student for a Certified Academic Institution, should not receive a Scantron Card. The student is to fill out the survey questionairre and mail the entire bi-fold form to RMCM Ministries for scoring.
