Sometimes when I try to have MinkSonic rename my MP3 files based on the artist & title, the filename doesn change. What am I doing wrong?

MinkSonic will not rename a file if the target filename already exists. This prevents accidental loss of data in the case where you have two versions (i.e. different bitrates) of the same song in the same directory.
