Cryptococcosis is a serious and lethal disease in AIDS patients. After initial treatment with Amphotericin B and or Fluconazole, chronic prophylaxis is indicated. Among the choices, itraconazole is suggested. DESCRIPTION: Case Report. RESULTS: A 40 year old male homosexual, while on successful disseminated Histoplasmosis treatment with itraconazole 200 mg QD developed headache and vomiting. A brain CT scan was normal. After 6 months of itraconazole treatment symptoms worsened and retinal examination showed signs of increased CSF pressure that prompted hospital admission. A spinal tap showed culture proved Cryptococcus Neoformans, glucose 20 mg/dl and normal cell count. A repeat brain CT scan was normal. Treatment was initiated with Amphotericin B that was poorly tolerated and continued with fluconazole only (400 mg QD). There was symptomatic imp
