Should I send a Child Information Form (green sheet) to the WECA food program office?

No, the information you used to write on the green sheet must now be emailed to WECA. This includes updates to child information (to drop a child from care, to correct a date of birth, etc.), a school-age child in care, holiday care, a change in provider information, infant food provided by parent, e-mail address change, and withdrawal from the food program. Under “What would you like to do?” on the CACFP.Net main menu page, click below the purple line, on “Click here to access forms on WECA’s website.” Select and complete the correct online form(s), the scroll down and click on Send.
