In the past the guidance has been for GP Practice receptionist to ask for the NHS Number from the patient who wishes to register with the new practice as part of their registration using the GMS1 form. A patient providing the NHS Number can be registered (including updating any demographics). The practice then informs the primary care registration back office, usually electronically though GP Links. A patient who is unable to supply an NHS number is asked to complete a GSM1 form with as much information as possible so that the existing NHS Number and associated records can be traced. Existing records will provide information about the patient’s previous GP and allow their previous medical history to be transferred to the new practice. In some instances when a practice has registered a patient without an NHS Number, the primary care back office has removed the patient and required the practice to get an NHS Number from the patient.
