Should applications only address residents in area-wide populations and population subgroup analyses and plans?
All activities supported through this FOA must be designed to either achieve, or build community capacity to achieve, area-wide health improvements and reductions in health disparities. Area-wide efforts should address all members of the community within the area to be served. This would include populations who may not have residency in the community (e.g., military staff, college students, etc.), but are part of the overall community make-up and could benefit from the strategies being put in place. Additionally, successful applicants will be required to identify population subgroups experiencing health disparities, select population subgroups to address, and identify specific outcomes to narrow gaps. Applicants will be accountable for both population-wide improvement and narrowing of specific gap(s) in health outcomes.
