Risk Services (ARS) – How do I get At-Risk Services?
You or your representative should contact the Department of Social Services Medicaid Intake Unit, either in person or by telephone, to request an At-Risk referral. A social worker will visit your home to assess your specific care needs. If you are determined eligible, a service plan is developed with your input to include the services you need. A social worker will continue to work with you to provide ongoing case management.
