• The program is a Preferred Provider plan. • You have a choice of five calendar-year deductible plans: $500, $1,000, $1,500 and $2,500 and $5,000. • Prescription drug coverage is available without a deductible, but with a 20% per prescription co-payment. The maximum per prescription co-payment is $100. Prescription co-payments count against a separate $2,500 out of pocket expense limit. • Services other than prescription drugs are subject to the selected deductible and then a 20% co-payment (40% for non preferred providers). • A non-prescription out-of-pocket expense limit each calendar year is equal to the deductible amount plus $1,500. If participants use non-PPO providers the non-prescription out of pocket expense limitation is $4,500.
