Requirement for a School Entry Immunisation Certificate for my child?
Once your child has received the four-year-old and all required infant injections, The Australian Childhood Immunisation Register (ACIR) will automatically post you a Child History Statement (CHS). The CHS can be used as proof of immunisation when enrolling your child in Primary School. If ACIR has an incomplete history of your child’s injections you should contact the organisation that immunised your child. If your child has received immunisations overseas, or you are a conscientious objector to immunisation or your child has a medical condition which prevents them from being immunised, then you will need to contact Hume City Council’s Health Department on 9205 2200.
