The immunization section of the “Health History Questionnaire” is intended to provide the Health Center with a record of your immunizations as part of your overall medical history. The Health Center refers to the Health History Questionnaire when providing medical care. The State of Indiana mandates that the University must collect immunization information for Diphtheria, Tetanus, Measles, Mumps, and Rubella from all new students, including intercampus transfer students new to the Bloomington campus. Indiana State law also requires the University to inform you of the risks associated with meningococcal disease (Meningitis) and the benefits of vaccination. All Students are required to confirm that they have read and understand these risks (see the link below for details). The University is required to monitor immunization compliance, provide notifications, and prohibit students from registering for future terms if they fail to comply.
