The items in column B of the record layout Excel file (TDI Helpinsure Data Call Record Layout and ZIP lists 3-14-08.xls) are intended to be column headings. The record layout is a guide or template to develop the sample rates for each of the more than 300 ZIP codes; please do not submit the data in an Excel spreadsheet.
