All RCRA LQG s must properly complete Site Identification Form and Form GM (Waste Generation and Management) for a Biennial Hazardous Waste Report to be considered administratively and technically complete. Furthermore, one Form GM must be completed for each waste (code) type generated at the site. Form WR (Waste Received From Off Site) should only be completed by sites that are permitted to receive hazardous waste as a RCRA Transport, Storage, Disposal Facility (TSDF). The completion of Form OI (Off-Site Identification) is not required.
