Further Info] On April 26, 1986, the Unit 4 reactor at Chornobyl*, near Kiev in the Ukraine, suffered the worst nuclear accident in history. A combination of poor design, almost nonexistent safety culture, and human error lead to an uncontrolled power increase that destroyed the core and dispersed radioactive contamination over a wide region of the Ukraine and Byelorussia.
