My physicians office can provide me with a print out of immunization records and physical examination information. Can I send in that document instead of the ASFH health form?
Your physician’s form can be attached to the ASFH health form but you still need to send in the ASFH health form, too. Simply complete the information on the camp form that are not on the phsycian’s form. You can leave the sections blank on the camp form of the information that is printed on your physician’s form. Please make sure the emergency contact information is completed on the ASFH health form.
