If your licence is damaged and/or broken you can obtain a replacement. You must request a licence replacement in writing and include the correct fee for licence replacement (click Schedule of Fees and Fines to ascertain the current licence replacement fee). If your licence was issued more than 5 years ago you must supply 2 colour passport size photos with your name clearly printed at the back. On receipt of the above documents and the correct fee, a replacement licence will be prepared and sent to your residential address within a week. You must ensure that you provide your current address if it is different to the address you had previously given to the Commission. Further, you are reminded that an advice of change of address must be received by the Commission within 14 days of the change taking place in accordance with the Notice of Conditions of a Gaming Industry Employee’s Licence.
