There are many problems that can cause this. Check temperature, ammonia, nitrite, nitrate, pH, and water hardness to make sure they are not out of the safe range for your fish. You should do so in that order also. Check temperature first, then ammonia, then nitrite, ect. If those are parameters are in okay areas for the fish, then check to see if the filter is working well, if the carbon is still okay, and if any other filter media is okay.
