In addition to the physician completing and signing the DA Form 5888 and signing the DA Form 7246, he/she will also complete and sign a DD Form 2792 Exceptional Family Member Medical Summary. This is the EFMP enrollment form. The completed and signed DD Form 2792 must be turned in to the Medical Records department. Ensure the Medical Records staff signs the bottom of page 2 this is your verification that you have submitted the forms as required. KEEP A COPY FOR YOUR RECORDS!
