The physician will complete and sign the DA Form 5888 and sign the DA Form 7246. The DA Form 7246 is for Health Clinic use only and will remain at the clinic. If the physician returns this form to you, simply keep it in your personal files. The completed DA Form 5888 must be returned to your personnel office for processing. KEEP A COPY FOR YOUR RECORDS!! It is the soldiers responsibility to hand carry the DA Form 5888 from the losing installation to the gaining installation.
