The employee must complete an Employee Enrollment/Change (722.9 KB) form (HCA 50-400) and the Student Certification/Change (59.5 KB) form (HCA 50-700) within 60 days of the date the dependent became a registered student. The Student Certification/Change form must be sent to the HCA for approval. If approved, the effective date will be on the first of the month following the date the form is received by HCA.
