The Aged Care Assessment Team in your local area can arrange for an assessment of your situation. Once this has taken place, the community health worker will: 1) arrange referrals for the provision of community services to support both you and your elderly father at home 2) assess your father’s level of care, and 3) provide advice about residential placement and respite for yourself.The local Community Health Centre will be able to provide advice and support to assist your father to remain safely at home, including home visits by community health nurses and other therapy staff.Your local council can also provide services in the home such as home help, respite, meals and personal care assistants to help with everyday activities such as showering. In addition, other organisations, as well as councils, can arrange for regular visits to day centers, outings, transport to carer groups and other facilities to assist you and your father.
