Every insurance company licensed to write the business of workers compensation insurance within the State of Wisconsin MUST complete and file the Acknowledgement Form. Insurance carriers who cease writing worker’s compensation are required to continue reporting until their calendar year premium and incurred losses do not exceed $100,000.
