My child needs to take medication during school hours. What is required in order to do this?
• A completed Medication Consent Form signed and dated by the doctor or licensed health care provider is now required for all prescription AND over-the-counter medicines (ie: Advil, Tylenol, cough medicine, allergy medicine, etc.). There is no longer a ten day medication policy in place regarding over-the-counter medicine. The consent form must give complete administration instructions. The parent must also complete and sign the top of the form. (Medication Consent Forms can be found on the website or in the school office) • A fresh supply of medicine delivered in its original packaging labeled with the child’s name. • Any change in medicine, dosage, or administration time requires a new Medication Consent Form to be given to the school. • If a child transfers between Union County schools, a new Medication Consent Form is needed to be filled out. • If an emergency medication (i.e. inhalers, epipens, or glucagon) is needed to be carried by the student in school or on the bus, the back o
