The enrollment visit ensures that the provider and office staff are educated on the VFC program requirements and have the appropriate resources to implement the VFC program requirements and that the necessary paperwork is completed including the provider enrollment form and provider profile. If the provider enrollment form and provider profile cannot be completed at the time of the enrollment visit a date should be agreed upon with the office when the competed paperwork will be returned to the VFC program. The enrollment visit would not include the administration of the VFC Site Visit Questionnaire.
