MSSP provides a nurse and case manager who meet with you to learn your needs for independent home living. Working together, you will develop a plan to meet those needs by drawing on existing community resources. In some instances, MSSP will directly purchase services for you. Your MSSP nurse and case manager will arrange for the assistance you have agreed upon and will check regularly to make sure that the services continue to meet your needs. Should your needs change, your MSSP case manager will alter your plan of care and its services to meet those needs.
