If a city health department is located within a county with a population greater than 500,000, eligible applicants must apply to serve the entire county including the city. A funded applicant is responsible for implementing the program within the geographic area of the county and must serve the entire county including any cities within the county. Fifty percent of implementation award dollars must be distributed to organizations throughout the county to extend the reach and impact of the program.
