You should complete the front side of The Ohio State University Graduate School Reference Form http://admissions.osu.edu/apps/pdfs/refer.pdf and request that your recommenders complete the back of the form, and attach it to the recommendation letter. These forms are not used with electronically submitted letters. Some programs have their own specific paper reference forms. Be sure to provide the direct contact information so the graduate program can contact your recommender via phone, mail, e-mail, or fax to ask questions. The completed reference forms and letters should be sent directly to your intended graduate program by your recommenders.
