A62. The evaluation must be consistent with NYS licensure and scope of practice guidelines. The Instruction Manual for SCREEN Form: DOH-695 (2/2009), item 32 notes that the psychiatric evaluation must be completed by one of the following: a board certified/eligible psychiatrist, Ph.D. psychologist, psychiatric nurse or MSW social worker.
