You can click here Certification then add 1 reinstate to your shopping cart. You must enter CPT number on second line of “Street’ field. You have the option to also mail in a $100 money order to the NPA office. Make sure to inidcate CPT number on money order. Also include current address, phone number, and SSN so that your information can be re-entered into our database system.
