Is there any way to print out a sheet for each patient that looks like page 22 in the Outcomes Program Manual, the cardiac outcomes info sheet?

There is no page that you can print that looks exactly like this one. But you can print a patient outcomes summary report which will list all of the values for two sessions. If you only have one session worth of data the other column will just remain empty.
