Is there any type of Catastrophic Protection provided by the plan?
Yes. Catastrophic Protection is provided. The Plan pays 100% of reasonable and customary charges for covered expenses in a calendar year, after out-of-pocket expenses in that year exceed $2,500 for participating providers or $3,000 for non-participating providers. The deductible is for Self or Self and family, per calendar year.
