Is there an application form that Plan Sponsors must use to apply under the VCP?
There is no application form applicable to the VCP; however, the IRS provides Sample Submission Formats in Appendix D of Revenue Procedure 2008-50, which facilitate the submission process. The IRS also provides a Checklist designed to assist the Plan Sponsor and their representatives in preparing a submission that contains the information and documents required under each of those programs. The checklist must be completed, signed, and dated by the employer, Plan Sponsor, or the Plan Sponsor’s representative, and should be placed on top of the submission.
