Is there a time frame in which the required documents must be submitted to the Medicare Contractor?
Yes, supporting documentation must be sent to the Medicare contractor when you submit your enrollment application or when requested by the Medicare contractor when you submit your enrollment application or when requested by the Medicare contractor. The signed and dated Certification Statement must be received by the Medicare contractor before the Medicare contractor will begin to process the enrollment application. The effective date of filing an enrollment application is the date the Medicare contractor receives the signed and dated (blue ink recommended) Certification Statement. Please Note: A Medicare contractor will not process an Internet enrollment application without having received the signed and dated Certification Statement.
