Is there a separate application form for School of Music admission?
Not for undergraduates. Your audition is your application. You will be given a brief audition report form to fill out that you will hand to the faculty at your audition. They will have been provided the information you give us on the Audition Request Form. Graduate students require the submission of other additional materials.
